Purpose: To allow for the fair and equitable review and distribution of Health Insurance Wellness Initiative Budgeted funds to member organizations.
Operation: Immediately following the official adoption of the budget, member entities may submit their “grass roots” proposal. This proposal shall be on an application form provided by the Fund Program Manager.
The Fund Program Manager reviews the application for completeness of Fund goals as they relate to the Fund and the needs assessment of the Fund in correlation to the developed funding guidelines. The Program Manager forwards the reviewed applications to the Fund’s Wellness Committee with a recommendation of acceptance or denial. The Fund’s Wellness Committee shall affirm or reverse the Program Manager’s recommendation. The affirmed applications shall be placed on the next regularly scheduled meeting for a formal vote of approval or denial. Following grant approval, expenses must be submitted with HIF payment voucher along with all receipts and appropriate documentation for consideration.
A. Submissions – Grant application approval periods: The Wellness Committee and Fund will approve grant applications at the January and July meetings. It is recommended that all Wellness grant applications should be submitted two (2) months prior to the committee for final grant application approval (November and May).
B. Applications – Over the Funding Limit – Any application received that requests funds over the funding limit shall only be funded at the maximum funding limit permitted by their insured lives count. If after all applications have been reviewed and no other late applications have been received, the additional funds may be distributed if funds remain in the Wellness Budgeted line. These additional funds must be requested by the submitting organization and will only be allocated after October each year if available.
Appeals: Member entities that have submitted a Wellness application within the timelines prescribed shall be permitted to appeal the action of the Executive Committee of the HIF Commissioners. The appeal must be in writing and addressed to the HIF Program Manager. The appeal will be reviewed by the Wellness and Claims Committee of the Executive Committee and a written response shall be provided within 10 days of the received appeal.
Funding: Member funding limits shall be annually set by the Executive Committee of the Commissioners during each annual Budget adoption.
